Designated Manager

Job Description: The Designated Manager is responsible for providing program management and oversight of the services provided. The Designated Manager is to ensure that the daily needs of the persons served are being met in a manner that enables the greatest degree of independence possible. The Designated Manager reports directly to the Executive Director. The Designated Manager supervises the Designated Coordinator and Direct Support Staff. In the absence of the Designated Manager, the Executive Director shall assume any and all delegated duties. The Designated Manager will assume the Designated Coordinator responsibilities if the Designated Coordinator has not met 245D qualifications until they are able to meet requirements. The DM will oversee all Supervised Designated Coordinator documentation and sign off on it.

Qualifications:

Education/Experience Requirements:

1) A baccalaureate degree in a field related to human services, and one year of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older

2) An associate degree in a field related to human services, and two years of full-time work experience providing direct care to persons with disabilities or age 65 and older

3) A diploma in a field related to human services from an accredited postsecondary institution and three full years of full-time work experience providing direct care services to person with disabilities or persons age 65 or older

Must have a minimum of three years of supervisory level experience in a program providing direct support services to persons with disabilities or persons age 65 and older and meet minimum education requirements.

Additional Qualifications:

1) Maintain a cleared background study clearance through DHS

2) Maintain current certification in first aid and CPR training from a qualified source

3) Maintain medication administration certification provided by Riverview Services

4) Maintain a valid driver’s license and an acceptable driving record

5) Maintain a home telephone or cell phone for on call rotations

Responsibilities:

1) Maintain a current understanding of the licensing requirements sufficient to ensure compliance throughout the program.

2) Supervise and ensure the duties of the designated coordinator are fulfilled.

3) Ensure the program conducts an internal review of incident reports of alleged or suspected maltreatment.

4) Review incident/accident reports that do not require investigation. Complete forms or submit to Executive Director for investigation immediately.

5) Ensure the program implements corrective action identified as necessary by the program following review of incident and emergency reports according to requirements.

6) Evaluate the satisfaction of persons served by the program, the person’s legal representative, if any, and the case manager with the service delivery and progress towards accomplishing outcomes and ensuring and protecting each person’s rights.

7) Ensure staff competency requirements are met and ensure staff orientation and training is provided according to the requirements in conjunction with Human Resources Manager.

8) Ensure corrective action is taken when ordered by the commissioner and that the terms and conditions of the license and any variances are met.

9) Evaluate the information identified in #1-8 and develop, document and implement ongoing program improvements

10) Ensure services are provided in response to person’s identified needs, interests, preferences and desired outcomes as specified in the CSSP Addendum

11) Provide adequate supervision of staff providing direct support to ensure the health, safety, and protection of rights of each person and implementation of the responsibilities assigned in each person’s CSSP or CSSP addendum.

12) Oversee the use of positive behavior support strategies that produce positive behavior change of persons through formal behavioral programs and situational training

13) Provide information to regulatory agencies as necessary and requested by Administrator.

14) Ensure person’s funds are accounted for and appropriate records are maintained (count petty cash, balance checkbooks and review purchases)

Employee Related:

15) Participate in hiring process as needed

16) Monitor employee performance and implement disciplinary actions as necessary according to Riverview Services, Inc. policies.

17) Develop a biweekly employee work schedule including use of leave time while maintaining staff ratios according to licensing requirements.

18) Approval timesheets on a biweekly basis and submit to payroll dept. in a timely manner

19) Schedule and conduct employee performance evaluations at completion of training period and annually and recommend wage increases according to wage scale to administrator.

20) Schedule and conduct bi-monthly employee meetings including maintenance of meeting minutes and attendance record.

21) Conduct Designated Manager orientation with new employees

22) Be available to employees for emergencies in an on call basis during the week and on weekends as part of a rotation schedule.

23) Review site bills monthly or needed for accuracy

24) Visit potential referrals and make recommendations to administrator.

25) Ensure assigned AWAIR/Right-to-Know responsibilities are met.

Service Planning and Delivery:

1) Develop, document, and implement an individual abuse prevention plan prior to or upon initiating services.

2) Develop a Coordinated Service and support plan (CSSP) addendum within 15 days of service initiation based on the CSSP

3) Complete a self-management assessment prior to the 45 day meeting which produces information about the person that describes the person’s overall strengths, functional skills and abilities, and behavior or symptoms based on person’s status within the last 12 months at time of service initiation. Assessments must be conducted annually at a minimum and be reviewed by the support team as part of a service plan review.

4) Within 45 days of service initiation, must meet with the person, the person’s legal representative, the case manager and other members of the support team to determine the scope of services to be provided, desired outcomes and supports necessary to accomplish outcomes, person’s preferences, most integrated setting and how services are coordinated across other providers serving the person.

5) Within 10 working day of the 45 day meeting, develop a service plan that documents the service outcomes and supports based on the assessments.

6) Provide an opportunity for the person or person’s legal representative and case manager to participate in the ongoing review and development of the service plan and the methods used to support the person and accomplish outcomes

Service Plan Review and Evaluation

7) Meet with the person, person’s legal representative, and case manager and participate in service plan review meetings following stated timelines established in the CSSP or CSSP addendum or at a minimum of once per year.

8) Develop and maintain data to ensure ability to summarize person’s status and progress toward achieving outcomes

9) Must summarize the person’s status and progress toward achieving the identified outcomes and make recommendations and identify the rationale for changing, continuing, or discontinuing implementation of supports or methods in a written report at the time of the review meeting. If requested by the team in the CSSP or CSSP addendum the report must be sent out 5 days prior to meeting.

10) Within 10 working days of the review meeting, must send the CSSP addendum to the person, person’s legal representative, and the case manager. Within 10 days of the mailing must obtain dated signatures to document approval of any changes.

11) Must maintain record of current services provided to each person according to the requirements in 245D.095 sub.3. Thin charts on an annual basis.

12) Ensure access to the service recipient records for the person, the person’s legal representative, the case manager, staff providing services unless the information in not relevant to carrying out the CSSP or CSSP addendum.

Job Type: Full-time

Pay: $25.00 - $28.00 per hour
