Job Title: Health Care Coordinator / Resident Care Coordinator (HCC/RCC)
Job Summary:
The HCC/RCC is responsible for coordinating and overseeing resident care services to ensure compliance with state regulations (WAC) and company policies. This role includes leadership of the care team, on-call responsibilities, occasional floor coverage as a med tech/aide, and active monitoring of resident status and staff performance.
Key Responsibilities:
- Lead, train, and support care staff to ensure quality resident care.
- Oversee medication management and PointClickCare (PCC) documentation.
- Monitor residents for changes in condition and ensure timely interventions.
- Ensure compliance with WAC regulations and facility policies.
- Participate in care plan development and family communication.
- Cover shifts as needed to maintain safe staffing levels.
- Serve as part of the on-call rotation for emergencies or staffing needs.
Qualifications:
- Experience with medication management and PCC preferred.
- Strong leadership and communication skills.
- Ability to work flexible hours and respond to on-call needs.
- Knowledge of state regulations (WAC) and resident care best practices.

Pay: $26.00 - $27.00 per hour
