As the Field Operations Specialist you will be responsible for conducting service coordination and home visits for the care management team. You will be responsible for the clerical, administrative, customer service and coordination of activities to support care management.
Key Responsibilities:
- Travels to members’ homes and conducts in-home care management visits
- Communicates effectively with members/caregivers and Plan Team (clinical and non-clinical)
- Accurately documentations and uses the electronic medical record
- Schedules care management visits timely and effectively.
- Submits timely and accurately for travel reimbursement expenses
- Reads reports timely and manages the queue to schedule care management visits in a timely manner
Qualifications:
- High School Diploma (or General Education Degree (GED) required. BA/BS recommended.
- 2-3 years in a managed care or home care setting. Valid NYS driver’s license and the ability to visit members in their home.
- Must be able to read, write, and speak English. Bilingual required – Spanish.
- MS Office required.
- Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the business community.
Compensation:
- Salary: Min. $24.0384 hourly - Max. $26.4423 hourly
