Overview

The Intake Referral Coordinator supports dementia care and specialty program services by performing intake and processing referrals for the Martha W. Goodson Center, including the Geriatric Assessment Clinic, Driving Evaluation, and applicable federal and state programs. In partnership with clinically trained team members and under clinical guidance, this role utilizes a structured, patient and caregiver centered approach to collect required data for program-specific assessments, review referrals, and determine eligibility based on established criteria, while coordinating access to appropriate clinical, community, and program-based services.

What You Will Do

• Performs intake and manages the end-to-end referral process for Martha W. Goodson Center services and programs by collecting and reviewing required information, determining referral appropriateness based on established criteria, and coordinating service delivery through appropriate team assignment. Facilitates communication and information sharing across stakeholders, arranges transitions to external agencies when referrals are not accepted, and ensures consistent follow-up and engagement with all parties until services are initiated, completed, or closed.
• Utilizes clinical knowledge to gather and document accurate, program-specific referral information while effectively managing incoming inquiries in alignment with service excellence standards. Serves as a knowledgeable resource and backup support on available services, including eligibility criteria, access points, and alternative options, and communicates this information clearly to patients, caregivers, and referral sources while actively participating in interdisciplinary team meetings to support coordinated, patient-centered care delivery.
• Maintains data integrity and accuracy by entering, updating, and retrieving information across multiple systems, including timely documentation in designated medical record systems. Ensures all required records and forms are completed in accordance with organizational standards to support efficient operations, compliance, and continuity of patient care.
• Maintains a comprehensive and current knowledge base of Martha W. Goodson Center services and programs, relevant healthcare regulations, and community resources to support effective referral coordination and patient access to care. Actively stays informed of updates impacting service delivery and collaborates with internal teams and external partners to strengthen community relationships and support coordinated, patient-centered care.
  
  

Qualifications

Education

• High School Diploma or GED, (Required)
• Program Graduate, Nursing or Allied Health (Required)
• Associates Degree, Nursing or Allied Health (Preferred)
  
  

Experience

• 2 years Healthcare experience in related field (Required)
  
  

Licenses and Certifications

• CPR/BLS Certification - American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) within 30 Days(Required)
• Licensed Practical Nurse (LPN) - Virginia Department of Health Professions (VDHP) (Required) or
• License Physical Therapy Assistant (PTA) - Virginia Department of Health Professions (VDHP) (Required) or
• Licensed Occupational Therapy Assistant (COTA) - National Board for Certification in Occupational Therapy Inc (NBCOT) (Required)
