As a Pharmacist Med Recon, your work plays an important role in the care of patients and healthcare providers by assuring the safe and the effective use of medications as needed. - You will have daily interaction with your pharmacy team, nurses, physicians, and other facility staff in the hospital and intervene in conflicts between the Pharmacy and medical staff regarding drug therapy issue resolutions that may arise.
- You will provide medication information and consultation on pharmacokinetics, dose adjustments, laboratory test orders, and medication monitoring according to established policies and procedures.
- You will have the opportunity to participate in the training of new pharmacists, pharmacy students, and technicians.
- B.S. or PharmD from an ACPE-accredited School of Pharmacy
- Active pharmacist license in good standing with the California State Board of Pharmacy
- (PGY1) Pharmacy Practice Residency Preferred
- (PGY2) Specialized Residency in area of specialty Preferred
- National Provider Identifier (NPI) Preferred
- 3-5 years hospital or retail pharmacy experience required
- 1-year supervisory experience preferred
