Responsibilities
- Conduct thorough patient assessments, including vital signs, medical history, and physiological evaluations, to develop individualized care plans.
- Coordinate and facilitate discharge planning, post-acute care, and outpatient services to ensure continuity of care across settings.
- Manage utilization review and management processes by evaluating medical necessity and optimizing resource use within managed care frameworks.
- Collaborate with interdisciplinary teams—including physicians, social workers, and specialists—to develop comprehensive treatment strategies.
- Monitor patient progress through diagnostic evaluations, medical records review, and ongoing case management activities.
- Provide patient education on health conditions, medication adherence, and self-care strategies to promote engagement and compliance.
- Maintain strict adherence to HIPAA regulations and ensure accurate documentation within electronic medical records systems.
Requirements
- Valid RN license with current registration in the state of practice.
- Critical care experience with a focus on hospital medicine or emergency medicine settings.
- Experience working in Level I or Level II trauma centers, ICU units, or emergency rooms is highly preferred.
- Strong knowledge of infectious disease care, geriatrics, behavioral health, and post-acute care environments.
- Proficiency in medical terminology, physiology knowledge, and patient assessment techniques.
- Demonstrated ability to manage complex cases involving inpatient, outpatient, pediatric (PICU), or clinic populations.
- Excellent communication skills with a focus on patient service and education.
- Familiarity with healthcare regulations such as HIPAA and experience with medical records management.
