Job Overview
River Brook Healthcare Center is seeking a compassionate and dedicated Physical Therapist (PT) to join our in-house rehabilitation team. We are hiring Full-Time, Part-Time, and PRN (Per Diem) Physical Therapists to provide high-quality rehabilitation services in our skilled nursing and subacute rehabilitation setting.
As a Physical Therapist, you will evaluate residents, develop individualized treatment plans, and provide evidence-based interventions to improve mobility, strength, balance, endurance, gait, and overall functional independence. You will play a key role in helping residents recover from illness, injury, surgery, or hospitalization while enhancing safety, independence, and quality of life.
You will collaborate with an interdisciplinary team committed to resident-centered care and achieving optimal rehabilitation outcomes.
Essential Duties and Responsibilities

Perform comprehensive physical therapy evaluations and develop individualized treatment plans based on clinical findings, physician orders, and resident goals

Provide skilled physical therapy interventions to improve strength, balance, mobility, gait, transfers, endurance, coordination, and functional independence

Evaluate and treat residents with orthopedic, neurological, cardiopulmonary, and medically complex conditions
-
Monitor resident progress, reassess outcomes, and modify treatment plans as appropriate

Complete accurate and timely documentation, including evaluations, daily treatment notes, progress reports, recertifications, and discharge summaries in accordance with Medicare, Medicaid, state, federal, and facility guidelines

Collaborate with physicians, nursing staff, occupational therapists, speech-language pathologists, dietitians, and other interdisciplinary team members to coordinate resident care

Educate residents, families, and caregivers on therapeutic exercises, mobility techniques, fall prevention, home safety, and assistive device use
-
Participate in interdisciplinary care planning meetings, family conferences, and discharge planning
-
Recommend and train residents in the use of assistive devices and adaptive equipment
-
Maintain compliance with all professional, regulatory, and facility policies and standards
-
Promote a positive, resident-centered rehabilitation environment focused on quality outcomes and clinical excellence
Qualifications
Required
-
Graduate of an accredited Physical Therapy program
-
Current Georgia Physical Therapist (PT) license in good standing
-
Current CPR certification or willingness to obtain upon hire
-
Strong clinical assessment, communication, and documentation skills
-
Ability to work effectively within an interdisciplinary healthcare team
Preferred
-
Experience in skilled nursing, subacute rehabilitation, long-term care, or inpatient rehabilitation

Familiarity with Medicare documentation and reimbursement guidelines
-
Experience working with geriatric and medically complex populations
-
Knowledge of evidence-based rehabilitation practices and functional outcome measures
