Duties
- Conduct thorough patient assessments, including vital signs, physical examinations, and medical history reviews to determine clinical needs.
- Administer medications and injections, following established protocols and safety standards.
- Utilize electronic health record (EHR) systems for accurate documentation of patient encounters and medical records.
- Manage acute pain cases and coordinate appropriate follow-up care or referrals.
- Support diagnostic evaluations through specimen collection/processing and basic medical imaging techniques like sonography.
- Collaborate with multidisciplinary teams on discharge planning, case management, and utilization review to optimize patient care pathways.
Skills
- Proven experience working with diverse populations including pediatrics, geriatrics, assisted living residents, hospice & palliative patients, and individuals with developmental disabilities or memory care needs.
- Strong proficiency in EMR/EHR systems & familiarity with CPT coding, ICD-9/10 coding is highly desirable.
- Ability to perform complex procedures like cardiac catheterization assistance or neonatal ventilator management when required.
- Excellent patient assessment skills with a solid understanding of physiology and anatomy to support diagnostic evaluation across inpatient and outpatient settings.
- Effective communication skills for teaching hospital experiences or providing health coaching; experience working with individuals with disabilities or in post-acute care environments is a plus.
