What are the responsibilities and job description for the Case Manager - Adult position at Riverbend Center for Mental Health?
The In-Home Case Manager provides case management, care coordination, and supportive services to individuals and families receiving services through the Center. This position delivers services primarily in the consumer's place of residence, and in community settings to reduce barriers to care, support recover, and improve overall well-being. The Case Manager wors as part of a multidisciplinary team to ensure per-centered, trauma-informed, and culturally competent care. This position requires a bachelor's degree preferably in a social services field. Must have a valid driver's license, clean driving record, and reliable transportation.
