A skilled nursing home facility, located in Elgin, IL is currently seeking an experienced RN MDS/Care Plan Coordinator. In this role, the MDS Coordinator is responsible for planning, organizing, developing and directing the MDS assessments and care plans in accordance with current applicable federal, state and facility standards, guidelines and regulations. This position is responsible for appropriate reimbursement through accurate completion off the MDS. This person will also develop, implement and supervise the restorative nursing program and coordinates the corresponding programs.
Responsibilities include:
Review MDS and care plan data to ensure completion
Conduct initial assessments and develop, implement, and supervise the restorative program and plans of care
Review all admission assessments, transfer information and hospital and discharge summaries to determine criteria for Medicare admission
Responsible for Medicare monitoring admissions and discharges
Job Requirements:
Active IL RN licensure preferred; MDS and rehabilitation credentials
2 – 3 years’ applicable experience as RN and 1-3 years’ experience in MDS; preferred in long-term care
Ability to maintain a high level of confidentiality
Energetic, professional with exceptional interpersonal skills
