JOB SUMMARY:

The Occupational Therapist contracted through our organization is responsible for implementing standards of care for occupational therapy services and adhering to all conditions outlined in the Service Agreement.

DUTIES & RESPONSIBILITIES:

• Assume responsibility for ongoing interdisciplinary assessment and development of individualized care plans in partnership with patients, representatives, and caregivers.
• Provide services as ordered by physicians in the plan of care.
• Offer patient, caregiver, and family counseling.
• Provide patient and caregiver education.
• Communicate with physicians and other healthcare practitioners related to the current home health plan of care.
• Evaluate patients' functional status and home environment for hazards or barriers to independent living.
• Develop treatment programs and establish goals for improved function.
• Train patients in the use of adaptive equipment, prosthetic and/or orthotic devices.
• Maintain appropriate clinical records and report any changes in patients' condition to the physician.
• Participate in quality assessment and performance improvement programs.
• Identify patient and family/caregiver needs for other home health services and make appropriate referrals.
• Instruct patients' family/caregiver and other healthcare personnel in the treatment regime.
• Supervise Certified Occupational Therapy Assistants.
• Instruct, supervise, and evaluate home health aide care when therapy is the only skilled service.
• Participate in discharge planning for patients.

JOB REQUIREMENTS:

• Must be licensed where the HHA is located if licensure is required.
• Must have graduated from an OT program accredited by ACOTE of AOTA or any successor organization.
• Must be eligible for or have taken the OT certification examination administered by NBCOT.
• Two years of clinical experience as an occupational therapist.
• Demonstrates good verbal and written communication and organization skills.
• Possesses and maintains current CPR Certification.
• Must be a licensed driver with an insured automobile in good working order.

Pay: $80000 - $100000 / year
