Essential Functions:
Client Care & Support

Assist nurses with health screenings, assessments, and vitals

Support coordination of physical and behavioral health services

Provide client education on wellness, medications, and symptom management

Support outreach, engagement, and follow-up services

Respond to on-call needs as required
Prescriber & Medication Support

Coordinate labs, refills, and prior authorizations
-
Assist with medication administration, including injections (as appropriate)

Support medication room organization, compliance, and inventory

Communicate assessment findings to nursing staff and prescribers
Team Collaboration & Documentation

Participate in a multidisciplinary treatment team

Maintain timely, accurate, person-centered documentation

Support quality improvement and ongoing professional development
Education and Experience:
- High School diploma or GED
-
Medical Assistant OR CNA/SRNA certification preferred

Experience in behavioral health or primary care settings a plus

Strong organizational and communication skills

Commitment to culturally responsive, person-centered care
Salary : $17 - $25
