What are the responsibilities and job description for the MAT Care Manager - Physician's Clinic position at RIVER VALLEY BEHAVIORAL HEALTH?
Job Summary:
The MAT Case Manager coordinates and supports patients receiving medication-assisted treatment for substance use disorders. They act as a liaison between patients, prescribers, nursing staff, and community resources to ensure continuity of care and promote long-term recovery.
Essential Functions:

Conduct intake assessments and develop individualized treatment and recovery plans.

Monitor patient progress, adherence to prescribed medications, and overall treatment goals.

Provide counseling, education, and referrals to supportive services (housing, employment, peer support, etc.).

Coordinate care with physicians, nurses, therapists, and outside providers.

Maintain accurate, timely documentation in compliance with program and regulatory standards.

Advocate for patients’ needs while promoting harm reduction and recovery-oriented approaches.

Ensures compliance with Federal, State, and local regulations and RiverValley’s mission policies, procedures and standards.

Completes all mandatory compliance, HIPAA and state and federal regulatory training as required.

Maintains an appropriate degree of confidentiality.
Education and Experience:

Requires a background in social work, counseling, psychology, or a related behavioral health field.

Bachelor's degree from an Accredited College in a Human Services field or a Bachelor's degree in a different discipline with two (2) years' experience working in a health care setting, behavioral health setting preferred.

Valid driver's license, reliable operating transportation, and proof of current auto insurance

Prefer experience working with children, families, and adults who experience complex emotional and behavioral difficulties which may present in crisis.
Knowledge and Skills:

Knowledge of substance use disorders and evidence-based treatment approaches.

Strong organizational and case management skills.

Ability to build rapport and maintain professional boundaries with patients.

Familiarity with community resources and support systems.
Salary : $22 - $24
