Job Summary

Responsibilities
- Perform comprehensive patient assessments, including vital signs, physical examinations, and medical history documentation using electronic health record (EHR) systems such as Epic and Cerner.
- Administer medications and injections accurately, including IV infusions, immunizations, and specialized treatments like botulinum toxin therapy.
- Manage complex clinical procedures such as catheterization, suturing, tube feeding, ventilator management, and sterile technique to ensure patient safety and comfort.
- Coordinate care plans with multidisciplinary teams in various settings including hospital medicine, ICU, trauma centers, outpatient clinics, long-term care facilities, hospice, and home care environments.
- Conduct diagnostic evaluations such as laboratory specimen collection/processing, imaging procedures (sonography), and diagnostic coding (ICD-9/10) for accurate medical records.
- Provide triage services by assessing urgent patient needs in emergency rooms or urgent care clinics; respond swiftly to acute pain management cases or infectious disease concerns.
- Support discharge planning and case management activities to facilitate seamless transitions of care across different healthcare settings.
Skills
- Extensive experience working with electronic health records (EHR/Epic/ Cerner systems) and medical documentation standards compliant with HIPAA regulations.
- Proficiency in performing advanced clinical skills such as IV insertion, phlebotomy, catheter care, ostomy care, and airway management.
- Strong knowledge of physiology, anatomy, pharmacology, and medical terminology relevant to diverse specialties including pediatrics, geriatrics, behavioral health, and critical care.
- Ability to handle complex cases involving trauma medicine (Level I & II trauma centers), neonatal resuscitation procedures (Neonatal ventilator), dialysis treatments, or hospice & palliative medicine.
- Skilled in infection control practices and aseptic techniques essential for surgical or sterile procedures in operating rooms or sterile processing areas.
- Experience working with special populations such as individuals with developmental disabilities or dementia; familiarity with MDS assessments for long-term care facilities.
- Excellent communication skills for patient education on health coaching topics like nutrition or chronic disease management; capable of working effectively within multidisciplinary teams across hospital or community settings.
