The Opportunity
We’re seeking an Integrated Care Pharmacist Provider to function as a billable medical provider under a Collaborative Practice Agreement (CPA). This is a dynamic role where you’ll collaborate directly with primary care providers and interdisciplinary teams to improve chronic disease management, enhance patient outcomes, and drive innovation in value-based care.

What You’ll Do
- Provide direct patient care for chronic disease management (hypertension, diabetes, asthma/COPD, obesity, smoking cessation).
- Conduct medication therapy management (MTM), reconciliation, and patient assessments.
- Adjust, initiate, or discontinue medication therapy under established protocols.
- Order, interpret, and monitor labs in alignment with CPA agreements.
- Educate patients and providers on disease states, lifestyle modification, and medication use.
- Build strong provider relationships and collaborate on care plans.
- Drive quality improvement, ensuring compliance with HRSA, 340B, and value-based care initiatives.
- Track and report clinical outcomes such as BP control, A1c improvement, smoking cessation, adherence, and therapy optimization.
- Support value-based care and population-health initiatives by identifying eligible patients and closing care gaps.
- Support program development, student/resident training, and expansion of pharmacist-led services.
- Contribute to the refinement and expansion of pharmacist protocols, CPA agreements, and new clinical service lines.
- Participate in clinical audits, QI projects, and semi-annual performance reviews.
- Serve as a clinical resource for helping patients overcome barriers to medication access, health literacy challenges, and social determinants of health.
- Engage in care‑team huddles and interdisciplinary rounds to ensure true continuity of care.
What We’re Looking For
- PharmD degree required.
- 1 years clinical pharmacy experience (ambulatory care, primary care, or population health preferred).
- PGY1 residency preferred.
- Direct patient care experience (FQHC or underserved populations strongly preferred).
- Strong working knowledge of chronic disease management, quality metrics, and value-based care.
- Active state pharmacy license in good standing.
- Excellent communication skills to counsel patients, collaborate with providers, and train staff.
- Tech-savvy with EMRs, care management tools, and pharmacy software.
- A collaborative leader who can build teams, mentor others, and drive operational excellence.
