The Health Screener will support a community health program by conducting standardized health-related social needs (HRSN) screenings for Medicaid members. Reporting to the Lead Supervisor and Program Director, this role will complete patient intake, communicate with patients empathetically, assess health-related social needs, and connect individuals with appropriate Social Care Network resources and enhanced HRSN service categories. Professional growth opportunities may include advanced screening responsibilities, Care Navigator, Lead, Billing, and Order Management positions.
Responsibilities
- Conduct standardized HRSN screenings for Medicaid members.
- Complete patient intake and health assessments.
- Identify health-related social needs and enhanced HRSN service categories.
- Communicate screening information clearly and empathetically.
- Support connections to Social Care Network resources.
- Provide services within a community health program setting.
Required Qualifications
- At least one year of experience in health screening.
- High school diploma or equivalent.
- Experience with patient intake and patient communication.
- Empathy when working with patients.
- Bilingual proficiency in Spanish.
- Experience conducting standardized HRSN screenings for Medicaid members.
- Knowledge of enhanced HRSN service categories.
