Position Overview:
As a Psychiatric Nurse Practitioner (Psych NP), you will provide comprehensive mental health care to patients across assisted living throughout Queens, Long Island, Brooklyn or the Bronx. This role focuses on delivering compassionate, patient-centered psychiatric services while collaborating closely with interdisciplinary teams.
The ideal candidate is empathetic, dependable, and passionate about improving mental health outcomes and overall quality of life for geriatric and post-acute care populations.
Duties & Responsibilities:
- Provide on-site psychiatric evaluations, diagnosis, and treatment for patients in assisted living, skilled nursing, and long-term care facilities.
- Conduct comprehensive mental health assessments, including cognitive evaluations, mood disorders, anxiety, psychosis, and behavioral concerns.
- Prescribe, monitor, and manage psychotropic medications, ensuring safe and effective treatment plans.
- Collaborate with interdisciplinary care teams, including physicians, nursing staff, social workers, and facility leadership to develop and implement individualized care plans.
- Monitor patient progress and adjust treatment plans as needed to optimize outcomes.
- Provide crisis intervention and behavioral management strategies when necessary.
- Maintain accurate, timely, and compliant documentation in accordance with organizational and regulatory standards.
- Educate patients, families, and facility staff on mental health conditions, treatment plans, and behavioral interventions.
- Ensure compliance with all state, federal, and facility-specific healthcare regulations.
- Support continuity of care through follow-ups, coordination with outside providers, and appropriate referrals.

Pay: $145,000.00 - $185,000.00 per year
