Onsite Care Coordinator

Rio Pecos Medical Associates – Roswell, NM

Position Summary

Rio Pecos Medical Associates is seeking a compassionate, organized, and proactive Care Coordinator to join our growing Care Coordination team. This full-time onsite role focuses on supporting pregnant and postpartum women in optimizing their health and wellness while also assisting with the daily operations of the Care Coordination program.

The ideal candidate will have 2 years of medical experience, strong organizational skills, and a passion for supporting mothers and families in the community. This position requires working closely with patients, clinic staff, and the Care Coordination team to ensure eligible patients are identified, enrolled, and supported throughout their care journey.

This is an in-office position with occasional home visits, so a valid driver’s license and reliable transportation are required. Proficiency with computers and electronic documentation is essential.

Responsibilities

Patient Identification & Enrollment

Pull and review daily clinic rosters to identify patients eligible for Care Coordination services.

Ensure all eligible patients are given an opportunity to enroll in the program.

Coordinate enrollment timing based on patient availability and clinic schedules.

Provide education to patients regarding Care Coordination services and available resources.

Care Plan & Patient Management

Maintain and manage an active caseload of approximately 30–40 enrolled patients.

Develop and maintain care plans focused on improving maternal and newborn health outcomes.

Conduct monthly outreach and follow-ups with assigned patients.

Provide education, support, and referrals to community resources when appropriate.

Roster & Program Management

Manage and maintain Care Coordination patient rosters for the primary clinic.

Assist with managing rosters from secondary clinic locations and ensure patients are properly assigned.

Assign patients to Care Coordinators on a weekly basis to ensure balanced caseloads.

Monitor enrollment opportunities and ensure patient engagement across clinic schedules.

Onsite Care Coordination Support

Coordinate onsite care needs for patients, including assisting with services such as maternity belt measurements and other care coordination support.

Work closely with remote Care Coordinators to facilitate patient needs during clinic visits.

Communicate with clinical providers and staff to ensure coordinated patient care.

Program Operations

Work closely with Care Coordination leadership to ensure accurate program numbers, enrollment tracking, and reporting metrics.

Assist with daily operational tasks to support the Care Coordination program.

Ensure documentation of assessments, care plans, and patient interactions is completed accurately and in a timely manner within the EHR.

Requirements

Minimum 2 years of medical or healthcare experience

Must reside in Roswell, NM

Valid driver’s license and reliable transportation

Proficient computer skills, including electronic health records and secure communication platforms

Strong communication, organizational, and time management skills

Ability to manage multiple responsibilities in a fast-paced clinic environment

Preferred Qualifications

Experience in maternal or community health

Bilingual (English/Spanish) a plus

Experience working with care coordination or population health programs

Performance Expectations

Maintain timely and accurate documentation for all patient interactions

Complete required patient outreach and monthly touchpoints

Maintain organized and accurate patient rosters

Ensure eligible patients are identified and offered enrollment opportunities

Collaborate effectively with the Care Coordination team and clinic staff

Job Type: Full-time
