What are the responsibilities and job description for the Community Health Worker (CHW) position at Rio Pecos Medical Associates, LTD?
Focus: Clinical Navigation, Advocacy, & Social Determinants of Health (SDOH)
Role Overview: The CHW is a frontline health professional and a vital "System Navigator" for our patients. While the Doula focuses on the traditional and physical birth experience, the CHW focuses on the Social Determinants of Health-identifying and removing the systemic barriers (housing, insurance, food, transport) that prevent healthy outcomes.
Core Responsibilities:
- System Navigation: Assist patients with Medicaid/HCA applications, WIC enrollment, and navigating specialty referrals.
- SDOH Screening: Conduct and document Social Determinants of Health (SDOH) screenings for all prenatal and postpartum patients to identify barriers to care.
- The Arrival Series (Systems Lead): Facilitate the "Health Education" components of the group curriculum, focusing on newborn safety, maternal warning signs, and navigating the healthcare system.
- Clinical Documentation: Maintain high-level, objective clinical records in the EMR to support NM Medicaid (HCA) billing for community health integration.
- Community Capacity Building: Act as the "warm handoff" between the clinic and community-rooted resources, like our doulas.
Mandatory Credentials:
- Certification: Must be (or be eligible to become) a NMDOH Certified Community Health Worker.
- Training: Completion of the 10-module Train NM program, or program approved here (https://www.nmhealth.org/about/phd/pchb/ochw/train)
- Safety: Current CPR or BLS Certification through the AHA and completed HIPAA Awareness training.
The Lane/Boundary: This role provides system-level advocacy and does not provide physical labor support or clinical medical assessments.
Pay: $14.00 - $16.00 per hour
