Care Coordinator- Maternal Health (Roswell, NM)

Rio Pecos Medical Associates is seeking a compassionate and organized Care Coordinator to join our growing Care Coordination team. This full-time role focuses on supporting pregnant and postpartum women in optimizing their health and wellness.

The ideal candidate will have at least 2 years of medical experience, live in Roswell, NM, and be passionate about serving mothers and families in their community. This is a in office position with occasional home visits , so a valid driver’s license and reliable transportation are required. Proficiency with computers and electronic documentation is essential.

Responsibilities:

• Support pregnant and postpartum women with care planning and health goal setting
• Provide education, resources, and follow-up to promote maternal and newborn wellness
• Complete and document assessments, care plans, and patient interactions
• Coordinate with clinical providers and community resources
• Conduct occasional home visits in the Roswell area

Requirements:

• Minimum 2 years of medical or healthcare experience
• Must reside in Roswell, NM
• Valid driver’s license and reliable transportation
• Proficient computer skills, including electronic health records and secure communication platforms
• Strong communication and organizational skills
• Comfortable working independently and remotely

Preferred:

• Experience in maternal or community health
• Bilingual (English/Spanish) a plus
