Universal Provider Service is seeking for a qualified provider/attendant to provide services as planned and maintain, strengthen, and safeguard the client in his/her own home environment. The attendant provides care, warmth, and support in accordance with the specified individualized service plan for the client.
Functions and Responsibilities:
· Assisting the client in his or her activities of daily living.
· Provide supervision of activities and assisting when necessary for the protection of client from physical harm.
· Perform daily tasks specified or authorized by the supervisor or individualized service plan.
· Reporting all significant changes in the client's circumstances to the supervisor within 24 hours of awareness of occurrence, or by the next working day. Such as hospitalization or any break of service whether intended or not.
· Consciously watching for safety hazards and acting immediately to remove the hazard or take other reasonable steps to remove the risk of danger.
· Responsible to clock in/clock out using the EW device and/or landline to enter daily codes for scheduled hours
· Responsible to call agency to verify codes have been entered properly and that visits are confirmed.
· Refrain from handling medications or giving medications to the client (only reminding)
· Perform all assigned tasks in compliance with the agency's policies, rules, and procedures.
Qualifications:
l. Must be at least 18 years of age
2. Must show proof that he/she is able to work in the United States.
3. Be competent to perform the tasks assigned.
4. Must not be a spouse or common law spouse of the client/member. Applicant will be excluded from being a paid attendant.
Job Type: Full-time
Pay: $10.60 - $13.00 per hour
