- Evaluate, develop, and implement individualized treatment plans
- Provide ongoing physical or occupational therapy services and reassessments
- Document therapy services accurately and in compliance with all regulations
- Work closely with the interdisciplinary team to ensure the best outcomes for residents
- Promote independence, mobility, and quality of life for every resident you serve
- Follow all facility policies, procedures, and professional standards
- Current Physical Therapist (PT) or Occupational Therapist (OT) license in Colorado or a valid compact license
- Passionate about improving residents’ quality of life through therapy
- Strong communication and documentation skills
- Experience in a skilled nursing or long-term care setting is a plus, but not required
