As we expand our footprint in Fort Bend and surrounding areas, we’re hiring a hands-on and compassionate Sales and Marketing Community Liaison to help grow our referral network while ensuring seamless transitions for new clients and caregivers.
About the Role
As a Sales and Marketing Community Liaison, you’ll be in the field daily—building relationships with referral sources, conducting assessments, and playing a vital role in the client onboarding experience. This unique position blends sales and marketing with client care coordination, making it ideal for someone who is equally passionate about relationship-building and delivering high-touch care experiences.
Key Responsibilities
Develop and maintain strong referral relationships with hospitals, ALFs, SNFs, physician offices, home health, and hospice agencies
Conduct 30 weekly in-person visits and drop-ins within your assigned territory
Accurately document outreach and referral activity in our CRM system (WellSky)
Perform in-home client assessments and assist with care plan development
Conduct caregiver hand-offs and ensure smooth shift transitions for new cases
Complete routine supervisory visits to monitor care quality and client satisfaction
Participate in community events, senior expos, and networking groups
Collaborate with leadership on territory strategy, messaging, and campaign execution
Provide daily visit reports, referral updates, and photos (internal tracking/social media)
Qualifications
2 years of experience in home care, hospice, home health, or healthcare sales/marketing
Strong communicator and relationship-builder with a warm, outgoing personality
Experience with client assessments and care coordination strongly preferred
Comfortable conducting home visits and interacting with seniors and their families
Organized, self-driven, and goal-oriented
Familiarity with CRM systems (WellSky experience a plus)
Knowledge of Fort Bend County’s senior care network is highly desirable
Reliable transportation, valid driver's license, and current auto insurance
Bachelor’s degree preferred, or equivalent experience in senior care/healthcare marketing
