Care Coordinator (In-Home Care)

Right at Home is seeking a compassionate, organized, and motivated Care Coordinator to join our growing team. This role offers a unique combination of client care, caregiver training, leadership, and care coordination.

As a Care Coordinator, you'll work closely with clients and their families while providing ongoing support, education, and guidance to caregivers. This is an excellent opportunity for someone who enjoys both hands-on care and leadership and wants to grow professionally within a supportive, team-oriented environment.

While some remote work is available, this position is primarily field-based and requires regular in-person visits with clients and caregivers. Participation in the on-call rotation, including one weekend per month, is required.

Responsibilities

• Conduct new client assessments
• Complete initial client visits and help ensure a smooth transition into services
• Conduct quarterly supervisory visits for clients
• Develop, maintain, and update individualized care plans
• Provide hands-on training and support to caregivers in the field
• Lead and support a small team of Care Leads

Qualifications

Required:

• Associate degree or higher
• Previous direct caregiving experience
• Strong organizational and communication skills
• Ability to prioritize tasks and manage multiple responsibilities
• Valid driver's license and car insurance

Preferred:

• Three years of management or supervisory experience
• Experience training, coaching, or mentoring caregivers

Pay: $22.00 - $24.00 per hour
