Job Title: Senior Care Advisor (Community Partnerships & Growth)

We are seeking a high-energy, results-driven Senior Care Advisor to drive referral growth and expand our presence in the community. This role is not a desk job—it is a field-based, relationship-building position focused on developing strong partnerships with hospitals, case managers, social workers, senior living communities, and community organizations.
As a Senior Care Advisor, you will serve as the face of our agency—educating referral sources and ensuring that individuals in need choose our services. You will play a direct role in increasing census by building trust, solving problems, and positioning our agency as the go-to provider for home care.

Key ResponsibilitiesReferral Growth & Business Development
- Actively develop and maintain referral relationships with:
- Hospital discharge planners and case managers
- Skilled nursing facilities and rehab centers
- Assisted living and memory care communities
- Physicians’ offices, hospice agencies, and Home Health agencies
- Conduct regular in-person visits, drop-ins, and scheduled meetings to build trust and stay top-of-mind
- Identify and pursue new referral opportunities within assigned territory
- Execute weekly outreach goals (calls, visits, presentations, follow-ups)
Community Presence & Education
- Host and attend community events, workshops, and networking opportunities
- Educate referral partners and families on home care services, payer sources (private pay, Medicaid, VA), and care options
- Position the agency as a solution provider, not just a service vendor
Client Conversion & Intake Support
- Respond rapidly to new referrals and inquiries
- Collaborate with internal team to ensure fast, seamless start of care
- Follow up consistently to convert leads into active clients
Relationship Management
- Maintain ongoing communication with referral partners to ensure satisfaction and retention
- Proactively resolve issues and remove barriers to referral flow
- Track and report referral activity, conversion rates, and key growth metrics
Qualifications
- Proven experience in outside sales, healthcare marketing, or business development (home care, hospice, or healthcare preferred)
- Strong relationship-building and communication skills
- Self-motivated with a track record of meeting or exceeding goals
- Ability to work independently in a fast-paced, field-based role
- Valid driver’s license and willingness to travel locally
