What You'll Do
- Provide skilled nursing care during home visits
- Assess patient needs and help create individualized plans of care
- Administer medications, treatments, and wound care as ordered
- Monitor patient progress and communicate changes with the care team
- Educate patients and families with patience, empathy, and encouragement
- Support patients in maintaining comfort, independence, and quality of life
- Complete documentation accurately and in a timely manner
- Collaborate with physicians, therapists, aides, and other team members to ensure coordinated care
- Current and active Registered Nurse (RN) license in Indiana
- Minimum of 1 year of nursing experience
- Home health experience preferred
- Strong clinical judgment and assessment skills
- A caring, dependable, and patient-centered approach
- Excellent communication and time-management skills
- Current CPR certification
- Valid driver’s license, reliable transportation, and auto insurance
- Part Time
