- Valid ID (Driver’s License, Passport, or State ID)
- Social Security Card
- Employment Authorization Card (if required by your Social Security card)
- Pre-Employment Physical Form – must include MMR, TB test, and Drug Screen
(We can provide our physical form if needed) - COVID-19 Vaccine Card (if you received it)
- Flu Vaccine (if you received it)
- Hep B Vaccine (if you received it)
- Two (2) Work Related References
