- Valid Photo ID – Driver’s License, State ID, or Passport
- Social Security Card
- Permanent Resident Card or Employment Authorization Card – if applicable
- Two (2) Professional References – please have their contact information ready, including name, phone number, and/or email address.
- Pre-Employment Physical – must include:
- MMR immunity documentation
- TB screening
- Drug screening
