Ridgewood Rehabilitation & Living Center is seeking a compassionate and experienced Licensed Social Services Director to join our leadership team. Our facility is dedicated to providing exceptional long-term care and rehabilitation services while enhancing the quality of life for every resident we serve.

The Social Services Director plays a vital role in supporting residents and their families by addressing psychosocial, emotional, and discharge planning needs while advocating for resident rights and promoting the highest level of independence and well-being.

Key Responsibilities

• Complete admission assessments and comprehensive psychosocial evaluations.
• Develop individualized care plans to address residents' emotional, social, and psychosocial needs.
• Coordinate discharge planning and assist with safe transitions of care.
• Provide support and counseling to residents and families throughout their stay.
• Serve as an advocate for resident rights while promoting dignity, independence, and quality of life.
• Collaborate with the interdisciplinary team to ensure comprehensive resident-centered care.
• Maintain accurate and timely documentation in accordance with state and federal regulations.
• Participate in care plan meetings, quality assurance initiatives, and interdisciplinary team meetings.
• Maintain knowledge of community resources and assist residents and families with accessing available services.
• Ensure compliance with all CMS, state, and facility regulations related to social services.

Qualifications

• Bachelor's degree in Social Work or a related human services field required (or qualifications meeting state regulations).
• Prior experience in long-term care or skilled nursing preferred.
• Knowledge of Medicare, Medicaid, discharge planning, and long-term care regulations.
• Strong communication, organization, and interpersonal skills.
• Compassionate, resident-focused, and able to work collaboratively with an interdisciplinary team.
