Job Summary
We are seeking a dedicated Licensed Practical Nurse to join Delhi Community Health Center-WellPath Solutions Behavioral Health.
-Provider Nurse
Position is Monday-Friday, 8:00 a.m.-5:00 p.m.
Duties
- Provide Intake, screenings, and administer injections as prescribed by healthcare providers
- Educate patients and families on healthcare procedures and self-care techniques
- Collaborate with the healthcare team to ensure quality patient care
-Set up referrals , help meet with National Quality Indicators and follow-up as indicated
-Scheduling return appointments and follow-up patient telephone calls
Qualifications
- Valid LPN license in the state of practice
- Clinic experience is a plus
- Strong patient assessment skills
- Ability to provide compassionate patient care
-Familiarity with Electronic Health Record
