Responsibilities

• Conduct thorough patient assessments, including vital signs, physical examinations, and health history documentation using Electronic Health Record (EHR) systems such as Epic or Cerner.
• Administer medications, injections, IV infusions, and perform catheterizations following sterile techniques and safety protocols.
• Provide specialized care for patients with complex needs such as Alzheimer's disease, dementia, traumatic injuries from Level I or Level II trauma centers, and acute or chronic pain management.
• Assist with wound care, dressing changes, tube feeding, and ventilator management in accordance with physician orders and nursing best practices.
• Coordinate discharge planning, case management activities, and collaborate with physicians on diagnostic evaluations and treatment plans.
• Educate patients and families on health coaching topics including medication administration, nutrition, infection control, and disease management.
• Maintain accurate medical documentation compliant with HIPAA regulations and ensure timely updates of medical records and coding (ICD-9/10).

Qualifications

• Valid state licensure as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN).
• Proven experience in diverse clinical settings such as hospitals (including ICU or Emergency Room), nursing homes, hospice care, or home & community care environments.
• Strong knowledge of physiology, anatomy, medical terminology, and diagnostic evaluation techniques.
• Hands-on experience with EMR/EHR systems like Epic, Cerner, Athenahealth or eClinicalWorks is highly preferred.
• Skilled in patient assessment, vital signs monitoring, specimen collection/processing, phlebotomy, and injection administration.
• Ability to work independently in the field while maintaining excellent communication with healthcare teams.
• Critical care experience including ICU or trauma center exposure is advantageous; familiarity with ICD coding (ICD-9/10), CPT coding, and medical imaging enhances your effectiveness.
• Knowledge of infection control practices and aseptic techniques essential for safe patient care delivery.
• Exceptional organizational skills to manage multiple patient cases efficiently across various environments such as outpatient clinics or inpatient facilities.
