**POSITION IS OPEN UNTIL FILLED - FIRST REVIEW OF APPLICATIONS: JULY 13TH
Assists elderly clients or clients with disabilities by providing personal cares and assistance with minimal homemaking duties. May assist professional staff with community health outreach activities or events.
- Personal care, light housekeeping and meal preparation.
- Assists with and/or helps to teach clients to perform the activities of daily living.
- Helps clients with prescribed exercises and treatments.
- Assists clients with medications.
- Helps clients to plan, organize, and/or carry out household tasks.
- Helps clients to plan, organize, and/or carry out household tasks.
- Promotes client's mental alertness through conversation and other assigned activities.
- Helps clients to maintain a safe living environment.
- Reports any changes in the client's mental or physical condition or family situation.
- Daily charting on client care, and weekly checking for client assignments.
- Follows Agency policies regarding infection control and data privacy.
- Follows the plan of care for each client as determined by nurse case manager.
- Assists professional staff with community health outreach activities or emergency preparedness activities
High school diploma. Certified Nursing Assistant/Home Health Aide or Licensed Practical Nurse. (CNA/HHA must be currently licensed on the Minnesota Department of Health Nursing Assistant Registry). Valid Minnesota driver's license. One to three years' work experience in a related field.
PREFERRED QUALIFICATIONS:
Prior experience in Home Care or Long-Term Care Facility.
Bilingual (English/Spanish or English/Somali).
HOURS: Variable. Typical work hours 20-30 hours per week. Monday - Friday from 8:00 a.m. - 3:30 pm
SUPPLEMENTAL INFORMATION: Non-Exempt, General Unit Union – Grade 5
