Job Summary
We are seeking a dedicated Licensed Practical Nurse (LPN) to provide high-quality patient care across various healthcare settings. The ideal candidate will possess strong clinical skills, proficiency with electronic health record (EHR) and electronic medical record (EMR) systems such as Epic, Cerner, and Athenahealth, and experience in diverse areas including acute care, geriatrics, long-term care, hospice, and community health. This role involves performing patient assessments, administering medications and injections, managing IV infusions, and supporting patient safety through vital signs monitoring and physical examinations. The LPN will work closely with interdisciplinary teams to ensure comprehensive care delivery in compliance with HIPAA and NCQA standards.
Duties
Conduct thorough patient assessments including vital signs, physical examinations, and physiological evaluations.
Administer medications, injections, IV infusions, and perform catheterizations following aseptic techniques.
Manage airway care, ventilator support, tube feeding, and wound suturing as needed.
Document all patient interactions accurately within EMR/EHR systems such as Epic, Cerner, or Athenahealth.
Assist with diagnostic evaluations including laboratory specimen collection/processing and medical imaging procedures.
Provide specialized care for patients with complex needs such as Alzheimer’s disease, dementia, traumatic injuries (Level I & II Trauma Center experience), infectious diseases, or post-acute recovery.
Support discharge planning and case management activities to ensure smooth transitions of care across inpatient and outpatient settings.
Collaborate with physicians in emergency medicine, ICU, pediatrics, neonatal ventilator management, or home & community care environments.
Educate patients on health coaching topics including nutrition, medication management, family planning, and chronic disease management.
Maintain sterile processing standards for procedures such as suturing or spinal taps while adhering to infection control protocols.
Qualifications
Valid Licensed Practical Nurse (LPN) license in good standing.
Proven experience with EMR/EHR systems including Epic, Cerner, Athenahealth; familiarity with ICD-9/ICD-10 coding and CPT coding preferred.
Extensive clinical background in areas such as critical care (ICU), emergency room (ER), medical-surgical units, geriatrics (including Alzheimer’s & memory care), hospice care, pediatric/neonatal units (including neonatal ventilator management), or outpatient clinics.
Knowledge of advanced nursing procedures including IV insertion/infusion therapy, phlebotomy/venipuncture, catheter care/management, sterile technique/suturing.
Strong understanding of medical terminology and anatomy/physiology relevant to diverse patient populations including individuals with disabilities or developmental delays.
Ability to work independently while collaborating effectively within multidisciplinary teams.
