Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
- Monitor patients’ health by taking vital signs and observing for any changes in condition or behavior
- Support medication administration following care plans and HIPAA regulations to ensure privacy and safety
- Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and cooking
- Maintain accurate documentation of care provided, patient observations, and any incidents or concerns
- Operate specialized equipment such as Hoyer lifts or resident’s personal assistive devices to ensure safe transfers
- Support clients with cognitive impairments such as dementia or Alzheimer’s through memory care techniques and engagement activities
- Perform light housekeeping tasks including cleaning living areas to promote a safe and hygienic environment
- Collaborate with healthcare professionals to develop and follow personalized care plans that meet each individual’s needs
- Provide emotional support and companionship to enhance social interaction and overall quality of life
Requirements
- Prior experience in home health, assisted living, nursing home, or group home settings is highly preferred
- Knowledge of patient observation, vital signs monitoring, medication administration, and HIPAA compliance
- Certifications such as CPR and First Aid are required; additional certifications in dementia or Alzheimer’s care are advantageous
- Ability to perform heavy lifting safely using equipment like Hoyer lifts when necessary
- Experience working with individuals with developmental disabilities, autism spectrum disorder, or other special needs is desirable
- Strong communication skills to effectively interact with clients, families, and healthcare teams
- Valid driver’s license and reliable transportation may be required depending on the position location
