About the Role
Are you a master puzzle-solver who excels at balancing complex logistics with high-quality client care?
As a Home Care Coordinator, you will own the operational success, care plan compliance, and scheduling logistics for a dedicated "pod" of up to 60 clients. You will manage your weekly calendar inside WellSky, ensure strict clinical and regulatory documentation, and build strong connections with clients and their families.
To help you maintain this balance, you are heavily backed by a robust administrative team. We strip away top-of-funnel and back-end tasks so you can focus on driving your pod's performance:
- A Centralized HR Recruiter handles all top-of-funnel hiring and schedules your final interviews.
- A Remote Compliance Coordinator manages initial background checks, document loading, and official incident filings.
- A Hybrid Field Assistant is dedicated to your pod 1-2 days a week to help execute routine 90-day QA home visits.
What You’ll Do (Weekly Breakdown)
- Scheduling & Logistics (40%): Optimize the weekly caregiver calendar in WellSky, rapidly resolve last-minute call-offs using automated text broadcasting, and monitor EVV compliance.
- Client & Family Management (30%): Conduct proactive telephone "pulse-checks" with families, manage hospital discharge transitions, and oversee your Field Assistant's route.
- Final Selection & Onboarding (15%): Conduct final 30-minute face-to-face culture-fit interviews with pre-vetted candidates and lead weekly new hire orientations.
- Field Operations (15%): Step into the field only for complex new client intakes or high-risk care disputes within your assigned territory.
The Weekly Rhythm
- Early Week: Launch the scheduling grid, review weekend incidents, and lock in 80% of open shifts.
- Mid-Week: Run assistant briefings, interview pre-vetted candidates, and manage local client care needs.
- Late Week: Finalize and freeze the upcoming scheduling grid, blast remaining open shifts, and set up the weekend schedule.
Qualifications & Skills
- Experience: 2–3 years in non-medical home care, healthcare scheduling, or human services management.
- Software: High proficiency with home care scheduling platforms (WellSky / ClearCare experience is heavily preferred).
- Skills: Strong logistical mindset for puzzle-solving under pressure and a sharp eye for accurate EVV, care plan, and compliance documentation
- On-Call: Ability to participate in a rotating on-call schedule (approximately 1 week per month).
- Travel: Must possess a valid driver's license and clean driving record for occasional field travel between office locations and client homes.
