Key Responsibilities:
- Conduct comprehensive assessments and develop individualized care plans.
- Provide skilled nursing care, including wound care, medication management, IV therapy, and chronic disease education.
- Monitor patient progress and collaborate with the care team to ensure optimal outcomes.
- Educate patients and caregivers on health maintenance and safety practices.
- Accurately document all care provided in compliance with agency, state, and federal regulations.
- Willing to do on-call rotations, holidays, weekend, or weeknight.
Qualifications:
- Active Registered Nurse (RN) license in Pennsylvania.
- Minimum of one year of clinical nursing experience; home health experience preferred but not required.
- Strong clinical and critical thinking skills.
- Excellent communication and documentation skills.
- Reliable transportation and valid driver’s license.
- Current CPR certification.
