Position Summary:
The Case Manager is responsible for providing individualized support services to clients enrolled in the residential substance use treatment program. This role includes assessing client needs, developing service plans, coordinating care, facilitating referrals, and supporting clients as they work toward recovery and reintegration into the community. The Case Manager collaborates with clinical and support staff to ensure continuity of care and adherence to treatment goals.
Key Responsibilities:
- Conduct comprehensive assessments and collaborate with clients to develop individualized service plans.
- Maintain accurate and timely documentation in accordance with agency policies, licensing standards, and confidentiality regulations (e.g., HIPAA, 42 CFR Part 2).
- Coordinate internal and external services such as medical, psychiatric, housing, employment, education, legal, and aftercare planning.
- Monitor progress through regular case reviews and modify service plans as needed.
- Participate in multidisciplinary team meetings and provide case updates.
- Advocate for client needs and assist in navigating systems of care.
- Facilitate or co-facilitate psychoeducational groups related to life skills, relapse prevention, or discharge planning.
- Assist clients in securing benefits, transportation, and other supportive resources.
- Collaborate with probation/parole officers, social workers, courts, and family members when appropriate and with consent.
- Support clients in discharge and aftercare planning, including referrals to outpatient services, sober living, and peer support.
- Maintain compliance with all agency, county, and state regulations and documentation standards.
Qualifications:
- High school diploma or equivalent required; bachelor’s degree in social work, psychology, or related field preferred.
- Registration or certification as a substance use disorder counselor (e.g., RADT, CADC, CATC) preferred.
- Minimum 1–2 years of experience working in substance use or mental health services.
- Knowledge of community resources, recovery models, and harm-reduction practices.
- Strong written and verbal communication skills.
- Ability to work independently and collaboratively in a team environment.
- Proficient in electronic health record (EHR) systems and documentation.
- Must pass background check, TB test, and drug screening (as required by state/facility policy).
Work Environment and Physical Demands:
- Residential treatment facility setting with some local travel required.
- Must be able to sit, stand, and walk for extended periods.
- Ability to manage crisis situations calmly and professionally.

Pay: $25.43 - $29.00 per hour
