Position Summary
The Program Director/Case Manager (PD/CM) oversees daily operations and compliance of the SAMHSA CHOICES grant while providing high-quality case management and participant support services. This role is ideal for someone who is organized, passionate about recovery work, skilled in documentation, and excited to help build and grow a federal grant program.
ResponsibilitiesProgram Director – 30%
- Oversee daily operations of the CHOICES Program
- Ensure compliance with all SAMHSA grant requirements
- Complete program performance reports and data tracking
- Provide supervision and support to CHOICES staff
- Coordinate with the Executive Director and accounting team for billing and allowable cost tracking
- Maintain positive working relationships with community partners and referral sources
- Attend community meetings, provider networks, and outreach events
- Ensure program fidelity, quality assurance, and adherence to 42 CFR Part 2 & HIPAA
Case Manager – 70%
- Provide direct case management to individuals participating in CHOICES services
- Conduct intakes, assessments, and individualized service plans
- Assist participants with accessing housing, mental health and substance use treatment, benefits, and employment resources
- Coordinate referrals and care navigation
- Document participant encounters accurately and timely in the EHR
- Facilitate or co-facilitate groups as needed
- Provide crisis intervention, safety planning, and participant support
- Engage participants using trauma-informed and recovery-oriented practices
Qualifications
- Associates’s degree in Human Services, Psychology, Social Work, or related field (BA/Master’s preferred)
- 2 years of case management, recovery support, or behavioral health experience
- Experience with federal or state grants (SAMHSA preferred)
- Strong writing and documentation skills
- Understanding of recovery, harm reduction, and trauma-informed care
- Ability to work independently and as part of a team
- Background check required
Preferred Skills
- Familiarity with 42 CFR Part 2
- Experience working with individuals with co-occurring disorders
- Knowledge of local Central MA resources
- Strong organization, communication, and leadership abilities

Schedule & Compensation
- Full-time, primarily Monday–Friday with some flexibility
- Competitive salary based on experience
