Essential Responsibilities
✔ Deliver and supervise direct nursing care per physician orders and nursing standards.
✔ Develop, implement, and update individualized care plans, particularly for transitional care residents.
✔ Administer medications and treatments accurately and timely; educate residents as appropriate.
✔ Monitor and document resident status, including physical, emotional, and behavioral changes.
✔ Maintain compliance with infection control protocols and safety standards.
✔ Communicate effectively with physicians, families, home health agencies, and team members.
✔ Promote and protect residents’ rights, dignity, and choice in a person-centered care environment.
✔ Conduct routine rounds on all units to ensure quality of care and staff support.
✔ Assist with resident admissions and ensure completion of all necessary documentation (UDAs, order checks, care plans).
✔ Perform narcotic/pharmacy audits and crash cart checks; ensure appropriate witnessing and documentation.
✔ Waste and return narcotics according to policy and pharmacy guidelines.
✔ Review recent falls to confirm incident reports are complete and interventions have been implemented.
✔ Ensure resident care plans are updated in response to condition changes or communication updates.
✔ Monitor and respond to secure messages, pending orders, and other care-related tasks in a timely manner.
✔ Manage daily staffing, respond to call-ins, and adjust schedules to maintain adequate coverage.
✔ Respond to call lights and assist with direct resident care as needed.
✔ Lock facility doors after 8:00 PM in accordance with safety procedures.
✔ Check fax communications regularly and ensure proper routing to the correct unit or nurse.
✔ Check medication room stock and restock supplies as needed.
✔ Support a professional team environment by fostering open communication, resolving issues, and encouraging professional growth.
✔ Remain calm and resourceful in emergencies, ensuring appropriate response and follow-up.
✔ Maintain confidentiality and adhere to all Resthaven policies and procedures.
✔ Perform other job-related duties as assigned.

Education, Training, & Experience
✔ Graduate of an accredited LPN or RN nursing program.
✔ Minimum of 3 years of nursing experience, preferably in long-term care or geriatrics.
✔ At least 1 year of supervisory or charge nurse experience.
✔ Ongoing participation in continuing education to maintain licensure and enhance clinical skills.
Physical Requirements
This role is active and hands-on. You must be able to:
✔ Stand, walk, bend, lift, push, pull, and assist residents throughout your shift
✔ Lift up to 50 pounds
✔ Use mechanical lifts and clinical equipment safely
✔ Maintain visual acuity and physical stamina required for resident care
