Role Description
This is a Part Time on-site role located in York, PA, for a Social Services Team Member. The role includes providing support to residents and their families through individualized case management plans, conducting assessments to address social and emotional needs, and collaborating with multidisciplinary teams. Additionally, the person will mainly work with the Long Term Care population but will also assist the Director of Social Services with new admission assessments and discharge planning as needed. Residents in the LTC area are visited at least quarterly with additional visit to address other needs or concerns as they arise.
Qualifications
- Experience in Social Work, Social Services, and Human Services
- Ability to communicate effectively and maintain thorough documentation
- Empathy and commitment to improving the well-being of individuals and families
- Bachelor's degree in Social Work, Human Services, or a related field is preferred
- Prior experience in a healthcare or rehabilitation setting is a plus
- Familiarity and interest in the older population is a must as well as working with individuals with dementia
