Reports to:  Investigations Manager  

Non-Exempt 

Duties and Responsibilities: 

1. Conducts audits and investigations as assigned.  

1. Performs job site and or workplace inspections to ensure compliance with Title XIX federal standards, state, local standards and or policy requirements.  

1. Conducts accident and or incident investigations and reports investigative results to appropriate staff with recommended corrective action.  

1. Responsible for leading the investigation review committee, conducting investigations in the stead of the Investigations Manager.  

1. Assists the QIM in monitoring and tracking the submission of all state required forms.  

1. Assists the QIM in the monitoring and completion of all investigations by the Investigator to ensure all investigations are completed and reviewed on time. 

1. Participates in the PI/RM/UR, Safety and Governing Body Committees. 

1. Reports to and provides updates to supervisor.  

1. Conducts Client Satisfaction Surveys quarterly. 

1. Conducts client chart audits quarterly. 

1. Updates PCM Debriefing Form as needed. 

1. Other duties as assigned. 

Knowledge, Skills and Abilities: 

1. Title XIX standards and their interpretation by federal surveyors of the Health Care Financing Administration 

1. Applicable consumer protection and incident investigation requirements and practices of all agencies of the State of Oklahoma; and 

1. Applicable accreditation standards for programs which serve consumers with dual diagnoses of intellectual disability and mental illness/behavior impairment/severe emotional disturbance.  

1. Word processing and data base applications 

1. Basic record keeping procedures. 

2. Developing outcome-oriented quality improvement plans and programs 

3. Quality assurance, quality improvement and/or quality management methodologies 

4. Interacting with clients, their family members, and diverse teams of clinical, direct care and administrative support personnel.

5. respond effectively to a changing environment specifically designed to support Greer Center Interdisciplinary Teams in their pursuit of consumer habilitation and community reintegration goals.

6. Operate standard Office Equipment 

7. Perform statistical calculation and produce descriptive summary data 

8. Communicate effectively both orally and in writing 

9. Use video, audio and other types of equipment to present trainings 

10. Gather information to use in reports 

11. Establish and maintain effective working relationships with others. 

12. Ability to stand, sit, bend, lift, climb, balance, stoop, kneel, crouch, and move intermittently.  Use hands to handle or feel.  Ability to turn on the knees, back, wrist, shoulders, and grasp with both hands.  Must be able to frequently lift/move up to 50lbs. and occasionally lift and move more than 50lbs.   

13. Must obtain and maintain current certification in First Aid and Cardiopulmonary Resuscitation (CPR), including abdominal thrust technique.  Training must be obtained through class attendance that includes hands-on instruction. 

14. Must obtain and maintain current certification of Professional Crisis Management training (PCM). 

Qualifications: 

1. Bachelor’s degree in related field preferred. Related college/university coursework and related experience will be considered in lieu of Bachelor’s degree 

1. One year experience in related field preferred (i.e., law enforcement, DHS, etc.) 

1. Demonstrated experience and expertise in quality assurance, risk management and auditing preferred. 

1. Certified investigator from an approved program or obtain certification within six (6) months of employment. 

Salary : $28 - $33
