The Support Coordination Supervisor oversees a team of Support Coordinators, ensuring excellence in person-centered planning, compliance with DDD regulations, and consistent, high-quality service delivery. This role provides supervision, coaching, and support in managing caseloads, navigating complex situations, and maintaining strong relationships with families and providers.
· Provide direct supervision, coaching, mentoring, and performance feedback to Support Coordinators.
· Conduct regular case reviews to ensure ISPs meet DDD expectations and reflect meaningful person-centered goals.
· Support ongoing staff training, professional development, and adherence to best practices.
· Assist in onboarding and supporting new Coordinators as they transition into the role.
· Ensure all Support Coordinators maintain compliance with DDD documentation standards, timelines, and policies.
· Review and monitor service plans, annual updates, and progress notes for accuracy and quality.
· Support internal quality assurance efforts and participate in audit preparation.
· Oversee caseload assignments, staffing coverage, and overall workflow needs.
· Assist Support Coordinators with facilitating Individualized Service Plan (ISP) meetings, annual reviews, and team discussions.
· Provide support with complex cases, crisis navigation, or barriers to service access.
· Promote self-advocacy, independence, and full community participation for individuals served.
· Build and maintain collaborative relationships with families, providers, community organizations, and DDD representatives.
· Track and monitor deadlines, compliance benchmarks, and caseload updates.
· Prepare reports for agency leadership and respond to DDD-required reporting needs.
· Participate in leadership meetings, contributing to program development and continuous improvement.
· Ensure timely and accurate documentation in DDD-approved systems (e.g., iRecord).
· Bachelor’s degree.
· 2–3 years of experience in support coordination, case management, or disability services.
· Strong familiarity with NJ DDD policies, Medicaid waiver services, and community resources.
· Excellent communication, organization, documentation, and problem-solving skills.
· Valid driver’s license and ability to travel for community visits.
· Completion of DDD Support Coordination Training (or ability to complete upon hire).
· Master’s degree in social work, psychology, human services, or a related field.
· Supervisory or leadership experience.
· Experience with person-centered planning and working with individuals with IDD.
· Compassionate, person-centered leadership style.
· Strong attention to detail and quality assurance.
· Ability to coach and support staff while maintaining accountability.
· Skilled in navigating complex systems and resolving service barriers.
· Culturally competent and able to build rapport with diverse individuals and families.
· Comfortable balancing administrative responsibilities with hands-on staff support.
· Community-based work, including home visits and provider meetings.
· Office, hybrid, and field-based responsibilities.
· Occasional evening or weekend meetings based on family or provider availability.
Pay: Up to $70,000.00 per year
