Join our team at the Short-Term Residential Therapeutic Program (STRTP) serving Transitional Age Youth (TAY) in San Leandro. This program supports youth with significant mental and emotional health challenges as they prepare for independent living and adulthood.
Position Summary
The Case Manager provides direct services to youth and families, including psychosocial assessments, treatment planning, coordination with county agencies, and participation in Child and Family Team (CFT) meetings. This role is part of a multidisciplinary team committed to trauma-informed care and individualized support for youth with psychiatric disabilities and other behavioral health needs.
Key Responsibilities
- Coordinate educational services and supports for each client
- Complete intake documentation, case plans, and assessments
- Provide referrals and linkages to community-based services
- Implement and monitor individualized treatment plans
- Facilitate and attend CFT and IEP meetings
- Coordinate visitation schedules and family engagement
- Maintain accurate case records and Medi-Cal documentation
- Support youth in developing life skills and managing daily challenges
- Administer treatment interventions and behavioral support
- Assist with medication self-administration (after training)
- Apply the Prudent Parent Standard for age-appropriate activities
- Ensure compliance with all agency and state training requirements
- Participate in supervision and team collaboration
Qualifications
- Bachelor’s degree in Social Work, Psychology, Child Development, or related field
- At least 9 semester units in child/family-related coursework or 18 months of relevant experience
- Coursework or experience working with culturally diverse populations
- Coursework or experience in child welfare
- Must be at least 21 years of age
- Valid driver’s license, reliable insured vehicle, and clean DMV record
- Clear TB test, health screening, and criminal background check
- Current CPR and First Aid certification
- Must pass Child Abuse Central Index check
