Responsibilities:
- Develop and manage comprehensive, individualized care plans, documenting interventions and measurable outcomes.
- Continuously evaluate patient progress, adapting care plans as needed.
- Educate patients and caregivers, promoting independence and self-management.
- Perform skilled nursing procedures using standard homecare equipment and supplies.
- Collaborate with interdisciplinary teams, participating in regular case conferences.
- Engage patients and families in goal setting and care plan revisions.
- Maintain accurate clinical records and reports.
- Communicate effectively with physicians, healthcare teams, and families.
Qualifications:
- Passionate about providing exceptional home healthcare.
- 1-2 years of recent RN experience (ICU, CCU, Med-Surg, Oncology, Gerontology).
- Active RN license in Maryland.
- Current CPR certification.
- Proficiency in IV therapy and wound care.
- Strong organizational and communication skills.
- Basic computer proficiency (Homecare Homebase EMR experience a plus).
- Valid driver's license, reliable transportation, and an acceptable driving record.
Schedule:
- Monday - Friday; 8:00am-4:30pm (No On-Call!) with occasional weekend rotation
Compensation:
- $89,856 - $125,000 per year
- Mileage reimbursement at the IRS rate
