Overview
We are seeking a dedicated and compassionate Occupational Therapist to join our healthcare team with Resolve Harmony. The ideal candidate will provide comprehensive therapy services across various settings, including inpatient, outpatient, home health, schools, and skilled nursing facilities. This role involves working with diverse patient populations, such as children, adults, and seniors, with a focus on improving functional independence and quality of life. The Occupational Therapist will collaborate with interdisciplinary teams to develop and implement personalized care plans that meet individual needs.
Duties
- Assess patients’ physical, cognitive, and emotional abilities to determine appropriate therapy interventions
- Assist patients with Activities of Daily Living (ADLs) to promote independence in personal care and functional tasks
- Develop, implement, and modify individualized care plans based on patient assessments and goals
- Provide hands-on therapy to improve motor skills, coordination, and strength
- Facilitate discharge planning by coordinating with families, caregivers, and other healthcare professionals
- Work with patients with autism, disabilities, geriatrics, pediatrics, post-acute conditions, and those in early intervention programs
- Manage case documentation accurately in compliance with healthcare regulations
- Educate patients and families on therapeutic exercises and strategies for ongoing improvement
- Collaborate with interdisciplinary teams including speech therapists, physicians, nurses, educators, and social workers
- Participate in patient care meetings and contribute to quality improvement initiatives
Requirements
- Valid licensure as an Occupational Therapist in the relevant state or region
- Experience working in skilled and long term settings
- Background in working with people with autism, disabilities, geriatrics, pediatrics, or post-acute care environments such as skilled nursing facilities or home health services
- Knowledge of physiology, anatomy, medical terminology, and case management principles
- Ability to assist with ADLs and develop effective care plans tailored to individual needs
- Experience in discharge planning and working within outpatient clinics or school environments is a plus
- Strong communication skills for collaboration with multidisciplinary teams and educating patients/families
- Compassionate approach to patient care across diverse populations including those in inpatient or outpatient settings
- Ability to adapt therapy techniques for different age groups and clinical conditions such as ICU or early intervention cases
