Outpatient Physical Therapist/Physical Therapist Assistant role providing 1-on-1 care within a senior living community (Assisted Living Facility) in Millis, MA with minimal travel.
All patients are treated on-site
Opportunity to grow into a full-time role as the community-based caseload develops
This is not traditional home health. All care is delivered within one community, allowing for an efficient schedule without excessive driving. You will provide outpatient-style care without the pressures of a high-volume clinic environment.
What You’ll Do
- Provide 1-on-1 outpatient treatment sessions in an on-site senior living community
- Implement treatment plans established by the supervising Physical Therapist
- Communicate with patients, families, and the care team
- Complete documentation using a streamlined system
- Participate in supporting the integration of therapy services within the community through consistent clinical presence

Schedule
- Part-time position with an anticipated caseload of approximately 10–15 treatment hours per week. Current patient volume is established and expected to continue growing through internal referrals, new resident move-ins, and ongoing community engagement. Additional hours may become available as the program expands.
Qualifications
- Active Physical Therapist Assistant license in Massachusetts
- Ability to work independently in a community-based outpatient setting
- Experience in outpatient or geriatric populations preferred
- Strong clinical judgment and communication skills
- Comfortable working in a growing practice environment
