Per Diem Physical Therapist role providing 1-on-1 care within a senior living community (Assisted Living Facility) in Weston, MA with minimal travel and no productivity quotas.
$55/hour (per-visit compensation in an on-site outpatient setting)
All patients are treated on-site

This is not traditional home health. All care is delivered within one community, allowing for an efficient schedule without excessive driving. You will provide outpatient-style care without the pressures of a high-volume clinic environment.
What You’ll Do
- Provide 1-on-1 outpatient treatment sessions in an on-site senior living community
- Complete evaluations and establish plans of care
- Reassess and update treatment plans as needed
- Communicate with patients, families, and the care team
- Complete documentation using a streamlined system
- Participate in supporting the integration of therapy services within the community through consistent clinical presence
This role includes a ramp-up period as caseload develops through internal referrals within the community. You will help establish a strong therapy presence within the community by delivering excellent clinical care and building positive relationships with residents and staff.

Qualifications
- Active Physical Therapist license in Massachusetts
- Ability to work independently in a community-based outpatient setting
- Experience in outpatient or geriatric populations preferred
- Strong clinical judgment and communication skills
- Comfortable working in a setting where caseload builds over time rather than starting at full volume
