Job Summary

Clinical Care Coordinator (LPN or Medical Assistant)

Resilience Recovery is seeking a compassionate, organized, and dependable Licensed Practical Nurse (LPN) or Certified Medical Assistant (CMA/RMA) to join our growing outpatient addiction medicine and mental health practice. This position requires an individual who is comfortable working with individuals recovering from substance use disorders and those receiving treatment for mental health conditions.

The ideal candidate is professional, nonjudgmental, detail-oriented, and capable of balancing direct patient care with administrative responsibilities in a fast-paced clinical environment.

Duties

  • Room patients and obtain vital signs.
  • Obtain and document medical histories, current medications, and allergies.
  • Administer injections as ordered, including long-acting injectable medication.
  • Assist providers with patient evaluations and office procedures.
  • Perform urine drug screen collections and specimen processing according to clinic policy.
  • Obtain blood specimens when trained and authorized.
  • Monitor patients for adverse reactions following injections or procedures.
  • Provide patient education regarding medications, treatment plans, and clinic policies.
  • Maintain patient confidentiality in accordance with HIPAA regulations.
  • Serve as a primary clinical point of contact for patients between appointments.
  • Return patient phone calls regarding medications, treatment concerns, and provider instructions.
  • Coordinate medication refills according to provider protocols.
  • Complete prior authorizations for medications and specialty treatments.
  • Communicate with pharmacies to resolve prescription issues, insurance denials, and medication availability.
  • Coordinate care with hospitals, detox programs, residential treatment facilities, counselors, therapists, primary care providers, specialists, and referral sources.
  • Review laboratory and diagnostic results for provider follow-up.
  • Track pending referrals, laboratory testing, imaging, and medication approvals to ensure completion.
  • Assist patients in overcoming barriers to treatment and maintaining engagement in care.
  • Support continuity of care following hospitalizations, detoxification, or residential treatment.
  • Order clinical supplies.
  • Maintain medication inventory and storage logs.
  • Assist providers with clinical workflow and documentation.
  • Follow infection control and safety standards.
  • Maintain HIPAA compliance and patient confidentiality.

Skills

  • knowledge of medical terminology, anatomy, physiology, pharmacology, and ICD coding standards
  • Proficiency with EMR/EHR systems for efficient documentation and electronic health records management
  • Strong patient service skills with the ability to communicate clearly and empathetically across diverse populations
  • Demonstrate empathy, professionalism, and respect toward individuals in recovery.
  • Build trusting relationships with patients while maintaining professional boundaries.
  • Be proactive in solving problems and coordinating patient care.
  • Possess excellent follow-through and attention to detail.
  • Thrive in a fast-paced outpatient environment.
  • Work collaboratively with providers, counselors, and community partners.
  • Maintain confidentiality and exercise sound clinical judgment.

Pay is negotiable and dependent on experience - pay listed is starting pay

Pay: $16.00 - $20.00 per hour
