Position summary: Consistently provide clients with assistance and encouragement to live successfully and independently within the community.
Summary of essential position functions:
Staff will model and foster strong working relationship with client and coworkers
Staff will adhere to all agency policies and procedures as written and directed
Staff will display an understanding of and have knowledge in administering client’s behavior plan
Staff will assist client with appropriate and required amount of Day services each shift
Staff will oversee all personal hygiene tasks; i.e. bathing, brushing teeth, shaving, toileting, laundry, etc. per shift
Ensure client(s) are appropriately groomed (all hygiene) and clothed daily
Staff will provide medication administration
Staff will assist in preparing all meals
Staff will complete all documentation; i.e. behavior tracking chart, sleep chart, service log, incident reports, med count sheet, MAR, physician visit sheet, menu chart, objectives, time sheet, mileage sheet, etc., correctly and timely as required, per shift
Staff will be responsible for client’s personal safety both in the home and on outings
Consistently ensure client success and wellbeing
Model and foster strong working relationship with client
Assist client(s) with completing daily household responsibilities
Ensure organization, maintenance, and cleanliness of the client's home.
Effectively and appropriately communicate pertinent information regarding client success and wellbeing to others, i.e., TCM, Human Resource Coordinator, and Guardian by following proper chain of command
Conduct and document monthly Fire and Weather Drills
Assist client with spending and retain all client receipts
Minimum requirements: Direct care experience, high school diploma or equivalent, valid driver’s license and proof of current automobile insurance
Abilities required: Direct Care Staff must display compassion and understanding in the workplace. Punctuality and dependability are absolute required abilities. Direct care must also demonstrate positive attitude, flexibility in schedule and be a team player. Each staff member will be expected to adhere to a strong ethical standard.
