Former or current hospital case manager for hospital outreach biz dev at psych residential program

Job Overview

We are seeking a experienced former or current hospital case manager in Los Angeles County to perform hospital outreach business development for a psychiatric residential program with locations in West Los Angeles, Downey and Sylmar. The program treats both Medi-Cal and privately insured patients who are stepping down from a psychiatric hospitalization. For this role, you will need to establish contacts with case management teams at various hospitals in the L.A. County area and visit them in person. Your goal is to establish a referral network for these hospitals to step down their patients upon discharge to our residential psychiatric program. To aid you in your success, our medical director will also be available to join you during your meetings with case management teams. You will often bring these case managers luch while you network. You must be able to travel betwen various hospitals. The position has a base salary and a generous bonus structure that rewards relationship building and referral network development

Duties

• Develop and implement outreach strategies to establish relationships with Los Angeles County hospitals, including behavioral health units, psychiatric hospitals and emergency departments
• Provide hospital case managers with in person education regarding benefits of our Medi-Cal mental health and private insurance psychiatriac programs
• Track referrals and provide weekly reports

Qualifications

• Proven experience as a hospital case manager or in hospital outreach business development within psychiatric or behavioral health settings.
• Extensive knowledge of hospital operations
• Excellent client communication skills
• Demonstrated understanding of social work principles related to community resource coordination in health services social work contexts.

Pay: $3,500.00 - $10,000.00 per month
